payments for unemployment insurance.

This is needed even if you are a sole proprietor who
has employees. Obtain this by filing with IRS form SS-4 (application for
Employer Identification Number) with the IRS. Obtain this number on or before

Unemployment Insurance:
The Oklahoma Employment Security Commission Act
provides that under certain conditions, payments of money may be made to
unemployed individuals from an unemployment compensation fund contributed to by
employers subject to the Act. Apply to the Oklahoma Employment Security
Commission. OES-1 Report is filed with Oklahoma Employment Security
Commission. Call 1-405-557-7138 for this application to set up quarterly
contribution for unemployment insurance.
